Referrals are usually made by Care Coordinators (Social Workers or Community Psychiatric Nurses). In some cases, Consultant Psychiatrists will have the role of Care Coordinator. It is important that there has been a discussion between the service user and Care Coordinator prior to referral, to ensure the appropriateness of the referral.
Referrals will only be accepted on the completion of a
service request form, which ensures we are provided with the appropriate information to enable us to provide a
quality service to the individual. It is also a requirement that any other relevant paperwork is attached (i.e. care plan and risk assessment). Any information we receive is treated as strictly confidential.
Following the receipt and acceptance of the referral form, an appointment will be offered to invite the service user to an introduction to the service.
